Abstract

Objective — to determine the possibility of using prepuce flaps in reconstructive urogenital surgery. Material and methods. We have joint experience in the use of prepuce flaps in more than 31 patients. As a rule, the flap was used for replacement urethroplasty. However, prepuce may be of significant clinical importance as a separate element in complex reconstruction of penis and other urogenital areas. We report two clinical observations to demonstrate this opinion. The first one is patient with recurrent hypospadias of anterior urethra (over 5 unsuccessful previous operations on the urethra). In the second case, there was a subtotal amputation of penile organ as a result of work accident. Results. A satisfactory functional and cosmetic effect of surgical treatment was achieved in all patients. Discussion. Prepuce flap is a good plastic material for reconstructive urogenital surgery. The disadvantage of this flap is a limited available area, that supposes the use of additional surgical techniques, plastic material and need for several surgical stages in some cases. Harvesting of this flap is characterized by multiple features and implies the use of precision technology. Conclusion. Prepuce flap is effective element for phalloplasty and urethroplasty. The technique is characterized by high incidence of favorable outcomes and may be used as an independent surgical measure or in combination with other various surgical methods.
